All expenses will be evaluated for the event in question in accordance with the information provided in this form. Submission of this form is only a request and DOES NOT CONFIRM the participation of NCWORSHIP in said event. Correct information is required to process this request. After review, a NCWORSHIP representative will make contact with you.
